Prime Care Coordination was created to help ensure that people with intellectual and developmental disabilities receive the best services possible. Prime Care offers complete support for each individual, taking the time to really get to know each person and their needs and goals in order to provide a very personalized approach to their care.
The process began several years ago as we, a group of local care providers, worked together to obtain an Article 44 license. This license allowed us to create iCircle to assist anyone needing Medicaid Managed Long-Term Care (MLTC) services. After nearly two years of providing managed care services, we realized the same approach could be used to help people with intellectual and developmental disabilities to obtain quality care coordination services. This led to the creation of Prime Care Coordination in 2017.
In an effort to be the best Care Coordination Organization possible, we are committed to the same things that matter to our partner agencies and the individuals and families we serve. Prime Care is committed to diversity; we work with a diverse group of partner agencies and serve a diverse population. We want to make everyone feel comfortable with the services they receive. The Prime Care team is also made up of a staff who will help those we serve feel most “at home.” The Regional Offices and Care Managers are local and active in their communities, continuously developing relationships with the agencies you work with on a daily basis. Prime Care is committed to a family-like environment, and we are all working toward the best care possible for each individual.
