Individualized Care Management

Prime Care Coordination is a care coordination organization (CCO) serving 8,600 members throughout 42 New York Counties. Prime Care supports individuals with intellectual and/or developmental disabilities, many of whom have multiple and complex needs, by developing individualized care plans focused on the individual’s personal goals and dreams.

Prime Care’s person-centered approach offers complete support for each person, identifying and coordinating the appropriate providers and services their members need to live their lives in the most meaningful way.

HEALTH HOME CARE MANAGEMENT CORE SERVICES

Comprehensive Care Management

  • Help you create your Life Plan and help you get it started
  • Review your current service needs, such as your providers and support services, your goals and your engagement activities
  • Provide you with education and support for health, wellness and decision-making

Care Coordination and Health Promotion

  • Provide you with education and support for health, wellness promotion and prevention programs
  • Coordinate and arrange for additional needed services and ensure you have what you need to follow your treatment plan

Comprehensive Transitional Care

  • Tell you, your family and your support team about networks with local practitioners and health facilities, including emergency rooms, hospitals and residential/rehabilitation settings
  • Help you and your family prepare for life changes, as well as changes in needed services

  • Use Health Information Technology (HIT) to ensure all your care is being coordinated to best support you
