Student and Family Education Specialist - Finger Lakes Region


The Student & Family Education Specialist is responsible for establishing and maintaining an ongoing relationship with school districts throughout the Prime Care Coordination (PCC) network and will serve as the primary liaison between PCC and school districts. The Student & Family Education Specialist will maintain a working knowledge of educational regulations, standards, and best practices related to special education programming as well as provide educational advocacy services to assist PCC enrolled families with necessary support.

If selected for this position, you will be responsible for:

  • Maintain and serve as a resource with working knowledge of IDEA, education regulations, standards, and best practices as related to students with I/DD diagnosis
  • Assist students and families with identifying and obtaining necessary services and supports within an educational setting through advocacy services
  • Facilitate communication and co-planning among service providers, regular education teachers, and special education teachers
  • Responsible for assisting with the creation and implementation of IEP and 504 plans
  • Ensure cooperative relationships with educational staff and CSE teams
  • Work in collaboration with PCC Directors of Enrollment and Enrollment Coordinators to establish OPWDD eligibility for qualifying students
  • Work in collaboration with Care Coordination team to develop viable plans for continuing services and supports prior to students transitioning from an educational setting
  • Develop and prepare reports as requested
  • Maintain and serve as a resource with a working knowledge of Medicaid and SSI standards for application and utilization, OPWDD programs, and community supports available to students
  • Identify topics and provide training and information to PCC employees regarding special education, educational system, and other related topics
  • Perform all other duties relevant to the position as requested.
  • Represent Prime Care Coordination on community planning, education or advocacy groups, boards, task forces as assigned
  • Assist and provide input in the development of policies and procedures pertaining to children or youth services

Minimum Qualifications:

  • Bachelor’s degree in education or related field with two years of relevant experience OR
  • A Master’s degree in education or related field with one year of relevant experience.

To be considered for this position, you must be or have the following skills or experience:

  • Knowledge of IDEA, educational regulations/requirements, and best practices as related to students with I/DD diagnosis
  • Knowledge of OPWDD and/or DOH program and service delivery system and regulations
  • Must be able to multi-task, establish priorities, and meet deadlines.
  • Ability to work independently and motivate others.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to act quickly, assess, and act accordingly in crisis situations
  • Possession and ability to maintain a valid NYS Driver’s License; a vehicle is required to perform the job
  • Some positions may require bi-lingual skills

Physical Requirements:

  • Ability to sit continuously
  • Ability to turn/twist upper body
  • Ability to use your hand for repetitive action and fine manipulating for the purpose of keyboarding
  • Must be able to travel throughout covered territories in Upstate NY as needed.
