Leadership Team

Prime Care Coordination is led by Kate Wagner. Kate previously served as Vice President of CDS Life Transitions, providing day to day direction and support to CDS Monarch, Unistel, and Warrior Salute Veteran Services.

Kate began her career at CDS Monarch in 2012 as a registered nurse for the organization, progressing quickly to assume senior management responsibilities. In her role as Vice President of Operations at CDS Life Transitions, Kate has been responsible for more than 400 employees and over a $35 million budget. Ms. Wagner has overseen the care and support of more than 1500 people with IDD, gaining expertise in administration and leadership, nursing and clinical services, facilities, quality management and driving systemic improvements. Kate is a Registered Nurse and recently received her Masters of Science and Health Administration. She has a keen focus on the delivery of services to the individuals and families Prime Care Coordination serves and appreciates the role of PCC in their lives.

Terria Thompson has worked in the field since 2003, helping to serve people with intellectual and developmental disabilities and ensure they receive the highest level of care. Terria began her career as a Direct Support Professional, but has acted as a Director of Person-Centered Services at CDS Monarch since 2009, giving her over 16 years of supervisory experience.

Terria has dedicated her career to caring for the individuals and has demonstrated strong leadership skills in building and developing strong programs. Terria has a Master’s degree in Public Administration and among many skills she brings to this role, has extensive experience with quality improvement, fiscal compliance, performance management, strategic planning, project management, and training and development.

Terria will be overseeing Care Coordination Operations as well as Benefits and Entitlements.

Patricia comes to Prime Care Coordination with many years of Compliance & Quality Improvement and Health Information Management Administration experience. Throughout her career, she has held leadership positions in these areas with the Department of Health, Office of People with Developmental Disabilities/Finger Lakes DDSO and CDS Life Transitions.

She joined Prime Care Coordination in May of 2020 as VP of Compliance and Quality Improvement/Chief Compliance Officer for Prime Care Coordination. Prior to this position, Patricia actively assisted PCC with their CCO application and readiness activities, including overseeing the ASO Quality services.

Patricia holds a Bachelor’s of Science degree in Health Records Administration from Daemen College, as well as national certification from the American Health Information Management Association. She has completed courses toward a Master’s in Public Health Administration from SUNY Brockport and is an active member of the Health Care Compliance Association.

As the Director of Care Coordination East, Emily is the director of Prime Care’s Southern Tier, Central NY, and Mohawk Valley Regions. She develops and oversees Care Coordination policies and procedures in those regions. Emily assists in developing and implementing comprehensive and person-centered approaches for Care Coordination services to the individuals and families Prime Care Coordination serves. She coordinates and networks with other organizations, agencies, and service providers to foster community collaboration in addressing the needs of individuals and families, effectively communicating Prime Care’s services, mission, and vision.

Emily earned a Bachelor’s degree in Social Work from Keuka College. Before joining Prime Care Coordination in June of 2018, she worked for 16 years at a voluntary agency, started as a Medicaid Service Coordinator (MSC), to MSC Supervisor, and Director of MSC.

Daniel has worked at Prime Care Coordination for over two years. He has a Bachelors Degree in Human Services from Empire State University and an Associates Degree in Business from Jamestown Community College.
